Treatment System Review - Community Questionnaire
Purpose of this questionnaire
The Crime & Drugs Partnership (CDP) commission and manage the performance of all the Drug and Alcohol Treatment Services in the City.We are committed to improve and modernise Drug and Alcohol Services and ensure that the individuals and the families of those who are experiencing problems with Drugs or Alcohol receive the very best treatment available.
The CDP is currently carrying out a review of how effectively our Drug and Alcohol Services are meeting the needs of all our communities.
We would like to gather as many views, opinions, and ideas as we can to directly feed into the review process.
The aim of this questionnaire is to provide an opportunity for the people of Nottingham to inform and shape the future of Drug and Alcohol Treatment in the City.
What the questionnaire covers
- Section one asks for some information about you which helps us to meet equality and diversity requirements
- Section two asks about your awareness of and views on drug and alcohol services in Nottingham
- Section three is an opportunity for you to provide us with any comments or feedback that we haven't considered
Confidentiality
This questionnaire is completely confidential and no personal information is collected that would enable you to be identified and you will not be put on any mailing lists!
How to complete the questionnaire
There are two ways to complete the questionnaire:
