Treatment System Review - Stakeholder Questionnaire

Purpose of the questionnaire

The Crime & Drugs Partnership is undertaking a review of the drug and alcohol treatment system in Nottingham.
The review will look at all drug and alcohol services in Nottingham in order to ensure we have a treatment system that is compliant with national guidelines and meets the range of needs of our drug and alcohol using population.
We are consulting with a wide range of people as part of the review and want to include the views of key stakeholders such as partner agencies.
This questionnaire gives stakeholders opportunity to feed their views into the review.

What the questionnaire covers

The questionnaire is split into 3 sections:
  • Section one asks for some information about you which helps us to meet equality and diversity requirements
  • Section two asks about your awareness of and views on drug and alcohol services in Nottingham
  • Section three is an opportunity for you to provide us with any comments or feedback that we haven’t considered


Confidentiality

This questionnaire is completely confidential and no personal information is collected that would enable you to be identified.

How to complete the questionnaire

There are two ways to complete the questionnaire:
  1. Complete a paper copy of the form and return it to any drug service in a plain sealed envelope or post it back by freepost (no need for a stamp) to the Crime & Drugs Partnership:

All questionnaires must be received by Monday 30th June.

If there are some questions you don’t want to answer then you can leave them blank, you can complete as much or as little of the questionnaire as you like.
